It is our desire to make your first, and every, visit, to our centers as pleasant and productive as possible. Since most first visits include a consultation and an examination, they provide a great opportunity for you to learn more about us while we learn more about you. While some treatments may be performed during your first visit, most procedures will require an additional visit.

To provide us with the information needed to begin addressing your dental needs, we suggest that you bring the following with you on your first visit:

Your referral slip and any x-rays you might have
Completed new patient form
A list of any medications you might be taking
Any insurance forms and information

CALL US at (305) 692-0017– Since every patient is unique, we suggest that you call our center before your first appointment to find out if there are any specific instructions or information about existing medical conditions that we might need. And, of course, if you have other questions, we’ll try to answer them as well.

IMPORTANT – Patients under the age of 18 must be accompanied by a parent or guardian for their consultation visit.
